Patient Portal Setup Instructions

Before you begin, please make sure that you have the following:

1. A connection to a Wi-fi network
2. Wi-fi password
3. Apple password

Step 1: Install FileMaker Go from the App Store

• Tap one of these two links from your iPhone:
  FileMaker Go 14: https://itunes.apple.com/us/app/filemaker-go-14/id981268415?mt=8# OR
  FileMaker Go 13: https://itunes.apple.com/us/app/filemaker-go-13/id675292600?mt=8
• Tap GET, and then INSTALL to install FileMaker Go App.
• It may take up to 15 minutes to install, so please wait until it finishes downloading to move on to Step 2.

Step 2: Download & Open MD2_Patients.fmp12

• Tap this link and wait until the file finishes downloading: MD2_PatientsPortal_NG.fmp12
• Tap it again to install the MD2 Patient Portal database on your iPhone.
• When prompted, select Open in FileMaker Go. Note: You may safely ignore the small screen that appears asking you to return to this document for Step Three. There is no longer a Step Three.
• After the application opens, tap "Device." A list of FileMaker files should appear.
• Tap the MD2_PatientsPortal file.
• Enter your Chart2 account name and password
• Tap Login

Finished! To access the database from now on, simply tap the FM Go icon on your iPhone and log in.

Troubleshooting Tips

If you receive a message stating that "Safari cannot run this script" you are probably attempting to use the old MD2 icon to access the patient portal. With the newest version of the iPhone iOS software, the old MD2 icon may or may not work. If you are experiencing difficulty with it, it is best to remove the icon and access the patient portal directly through FileMaker Go. To do so, tap the FileMaker Go icon on your phone and follow Step Two, substeps 4-7.
